We will fill out a reservation form containing the above information and have it ready for you to verify and sign. In addition, the "owners release" will be on the written  form for your pet's protection. Please read the release, below:

All pets must be free of external parasites or they will be treated at the owner's expense. In case of an emergency, I authorize the Animal Hospital of Casper staff to do whatever is deemed necessary for my pet. I understand that if I do not let the Animal Hospital staff know of a change of the pick up date or do not pick up my pet within 5 days of the above date, my pet will be considered abandoned. (In accordance with the Wyoming Veterinary Practice Act.)
